Impfhonorar

The Impfhonorar has become the reference number against which owners judge every other service they offer. Vaccination is the one service where both sides of the calculation are known, because the fee is fixed and the work is familiar. Revenue per appointment can therefore be set against staff minutes per appointment without guessing at either.

From September 2026 the fee is 11,00 euro, which replaces the 10,68 euro paid before. It is renegotiated between the payer side and the pharmacy side, so it should always be checked for the current season rather than taken from a page like this one. A pharmacy that plans a season around a figure from two seasons ago plans around the wrong figure.

The comparison itself is straightforward and unforgiving, because an appointment is far more than the injection. One pharmacist is occupied for the whole of it, since this work cannot be delegated:

1. Somebody takes the booking and schedules the appointment.
2. Eligibility and contraindications are checked before anything is prepared.
3. The consent conversation happens with the customer in person.
4. The vaccine and the materials are then prepared.
5. The pharmacist administers the vaccination itself.
6. The customer is observed for a period afterwards.
7. The documentation is completed for the record.

Set the fee against those minutes and a pharmacy has an internal floor. Any other service that occupies a pharmacist for a comparable stretch has to earn at least as much, or it is being subsidised by the dispensing side of the business and by the Packungsfixum that funds it.

Against that floor the other routes look different from each other. A pharmazeutische Dienstleistung pays a defined fee for a defined protocol, and the service officially called Standardisierte Risikoerfassung hoher Blutdruck pays 11,20 euro net. That is close enough to the vaccination fee that the deciding factor becomes how long the protocol actually takes rather than what it pays.

An apothekenübliche Dienstleistung has no fixed fee at all, so it is invoiced through Privatliquidation, the pharmacy sets the price itself, carries the payment risk and has to answer the VAT question. That answer depends on what exactly was performed and belongs with the pharmacy Steuerberater before the first invoice goes out. The vaccination fee is the number a pharmacy has to beat, but it is not a number anybody else is obliged to match.
